PresbyMAX suitability

Almost everyone has to expect their vision to deteriorate from around the age of 45. But this does not have to be the case. Although this age-related and therefore completely normal process cannot be stopped, it can be treated. So far, classic reading glasses or varifocals, sometimes intraocular lenses, contact lenses or the LASIK procedure have been used for treatment.

The minimally invasive PresbyMAX method offers a gentle and high-quality alternative. Over 10,000 successful treatments have already been carried out worldwide.

Even with advanced presbyopia, it is possible to achieve excellent results. It is therefore possible to dispense with reading glasses in future and even correct other visual defects if necessary.
The PresbyMAX procedure is also suitable for patients with presbyopia and another basic visual defect.

PresbyMAX treatment steps
PresbyMAX – laser eye method for the treatment of presbyopia

At EyeLaser, we only offer PresbyMax in combination with Femto-LASIK. This is a special technique that consists partly of monovision in combination with multifocality.

1 . The preparation

We first disinfect your facial area. You will be given a local anesthetic in the form of eye drops. Your eyelids are fixed. All of this is absolutely painless and painless. You will be asked to fixate on the green light above you throughout the entire procedure.

2. the cut with the femtosecond laser

Using the high-precision femtosecond laser, the eye surgeon makes an incision in the cornea to create a 0.1 mm thin lid – the flap. This is carefully folded back. You may feel a slight pressure on the eye and it will darken briefly.

3. correction with the excimer laser

The eye surgeon then uses the excimer laser. The computer-controlled laser is used to reshape the exposed cornea in just a few seconds to compensate for the visual defect. You will not notice anything except the sound of the laser and possibly the smell of the cornea. The surgeon controls the use of the laser through a microscope.

4th degree

In the final step, the flap is folded back. It adheres on its own and forms the body’s own protection for the cornea. If necessary, highly oxygen-permeable contact lenses will be inserted to protect against environmental influences, which will be removed the following day. That’s it!

After the procedure, there is initially a clear change for the patient. Vision is perceived as different than with glasses. Patients usually (but not always) experience 3 phases (neuroadaptation):

  1. Adaptation: everything is new, seeing is rediscovered
  2. The tolerance phase: you can see well but vision is still perceived as somewhat unpleasant
  3. The phase of acceptance: Yoptimal vision at a distance and close up interacting with the brain.

All these phases together should not last longer than 6 months.

PresbyMAX risks

In addition to the LASIK/femto-LASIK risks, there is also the possibility that the patient will not accept the slightly different eyes or profile. In this case, there is always the option of correcting the eyes for optimal distance (+ reading glasses).

If the cornea is not suitable for laser eye treatment, phakic lenses (ICL) can be inserted. An alternative is refractive lens replacement using premium implants, which can also correct presbyopia.

How your eye sees after PresbyMAX

Schematic representation and cross-section of the cornea after PresbyMAX treatment. The near zone enables vision without a visual aid. The intermediate zone favors good vision at medium distances, e.g. at the computer workstation. The distance zone ensures good vision at greater distances. The transition zone contributes to the stability of the treatment result.

The PresbyMAX laser eye treatment only takes a few minutes and does not touch or replace the sensitive lens of the eye. The modeling of the cornea, on the other hand, is achieved using fast laser pulses – precisely, gently and quickly.

The principle of the PresbyMAX method is similar to that of contact or intraocular lenses. The focal points of the eye are calculated precisely. For this purpose, the cornea is modeled for near vision (in the center) and far vision (towards the periphery). This results in good visual acuity at all distances – whether near or far. There are various treatment options for the PresbyMAX method, which we will be happy to explain to you individually.

Achieving visual acuity is comparable to the speed of the LASIK method and is therefore not long in coming. You can do without your reading glasses just a few hours after the laser treatment. Within a week, however, you will be able to see well in the distance and at medium distances. Complete, multifocal vision, on the other hand, requires several weeks before the overall quality of vision is fully restored.

What is PresbyMAX and how does it correct presbyopia?

PresbyMAX is a laser procedure used to correct presbyopia. The cornea is shaped in such a way that it allows for multiple vision zones – for both distance and near vision. The procedure is based on a multifocal laser profile applied with an excimer laser to restore vision. A monovision adjustment can also be made, where one eye is optimized for near vision and the other for distance vision.

Who is a suitable candidate for PresbyMAX treatment?

Suitable candidates for PresbyMAX are patients between 40 and 65 years of age who exhibit beginning or intermediate presbyopia. There must be no cataracts present, even in the early stages. Furthermore, sufficient corneal thickness is crucial to ensure a stable correction. As part of a comprehensive preliminary examination, we determine whether you are a candidate for treatment with PresbyMAX.

How does PresbyMAX work and is it painful?

The treatment is performed on an outpatient basis and usually takes only a few minutes per eye. First, a thin layer of the cornea is loosened using a femtosecond laser or mechanically (in the case of LASIK), then the excimer laser reshapes the cornea according to the PresbyMAX profile. The procedure is painless, as anesthetic drops are administered beforehand. Any slight irritation or feelings of dryness usually subside within a few hours.

How long does the healing process take with PresbyMAX?

The first improvement in vision is usually apparent the day after the treatment. The healing phase typically lasts a few days to weeks until vision has stabilized. During this time, slight fluctuations in visual acuity or feelings of dryness may occur, but these are easily treatable with eye drops.

Are there risks or side effects with PresbyMAX laser treatment?

As with any eye laser treatment, risks are generally low but possible. These include temporary dryness, glare, halos, or reduced contrast sensitivity, especially in the first few weeks. In rare cases, over- or under-corrections can occur, which can be treated if necessary. A careful preliminary examination and follow-up care are important to minimize complications.
